How to Compare ACA Marketplace Plans: A Step-by-Step Guide

Premiums are only a third of the story. A practical, jargon-free method for comparing marketplace plans on total cost, network, and drug coverage.

Comparing health plans by monthly premium alone is like comparing cars by the down payment. Here's the order that actually works, whether you shop through HealthCare.gov, a state exchange, or a marketplace like ours.

Step 1: Start from your subsidy, not the sticker

Premium tax credits are the single biggest lever — they can take a $1,400 family premium to $0. Estimate your household income honestly, include everyone who files on your tax return, and compare plans on the after-credit price. If a tool doesn't show subsidy-adjusted prices, it isn't really comparing plans.

Step 2: Add your worst-case number

Every plan has a maximum out-of-pocket — the most you'd pay in a truly bad year, on top of premiums. Multiply the monthly premium by 12 and add the max out-of-pocket: that's the plan's real ceiling. A cheap Bronze plan with an $18,000 family MOOP can cost more in a bad year than a pricier Gold plan.

Step 3: Check the deductible against how you actually use care

Rarely see a doctor? A high-deductible Bronze or HSA-eligible plan keeps monthly costs down and the HSA adds a tax break. Manage a condition, take regular prescriptions, or expect a big year (a baby, a surgery)? Mid-deductible Silver — which at some incomes carries extra cost-sharing reductions — or Gold usually wins on total cost.

Step 4: Verify doctors and drugs before you fall in love

Every plan publishes a provider directory and a drug formulary. Check your must-keep doctors, your hospital, and every prescription against the specific plan variant — not the carrier generally. HMO and EPO plans cost less but won't cover out-of-network care except emergencies; PPO-style plans trade higher premiums for flexibility.

Step 5: Then, and only then, compare premiums
